There's the classic disaster-comedy version: the goldfish that didn't survive the week, the houseplant left in your care that staged a slow, dramatic death, the group project where you were "in charge" and everything caught fire, figuratively or otherwise. Let the humor come from your own overconfidence.
Or go quieter and more honest: the time you said yes because you were afraid to say no, and spent weeks drowning under something too big for you, learning what your limits actually were. That's less about the failure and more about what it taught you regarding asking for help.
There's also the tender angle: being handed care of someone younger, older, or more vulnerable than you, and realizing responsibility isn't about perfection but showing up anyway, imperfectly, and staying.
