One way in is quiet realism: she waits for him to notice the new nose, the sharper jaw, and when he finally does, his compliments land like static instead of comfort, because she can't tell if he's praising her or a stranger wearing her name. Push it toward comedy and the surgery could go slightly, delightfully wrong, giving her a temporary look nobody expected, forcing the boyfriend to prove his feelings under absurd circumstances. Or tilt it speculative: the procedure is some near-future technology that lets her adjust her face like a dial, and the real story becomes about her chasing an ever-moving target of what "enough" looks like, with the boyfriend almost beside the point. Each version asks the same question differently — is she trying to be loved, or trying to stop being afraid.
